Creating my own PHP Web-Based Proxy?

Sorry if there is a topic already about this subject, but I'm not 100% sure what I'm looking for. I want to know if there is any tutorials on how make my very own proxy website. I'd also like to know if it would be possble to make it via DreamWeaver. Here are some sites:
